Our Approach to Lazy Eye Treatment Without Patching

At Visual Processing Institute, we use individualized vision therapy without patching. Our approach emphasizes helping the eyes and brain work together, with activities selected for the visual deficits identified during the examination.

Depending on the diagnosis, treatment goals may include:

• Encouraging the brain to use information from both eyes.

• Developing binocular coordination and depth perception.

• Improving measured eye-movement control.

There is promising research on specific binocular treatments. A randomized trial involving 28 children found that a binocular iPad game improved visual acuity by an average of 1.5 eye-chart lines after two weeks, compared with 0.7 lines with patching. Kelly et al., 2016

However, a larger trial involving 385 children found greater average improvement with patching than with a different binocular game. Results depend on the treatment studied; neither trial establishes that our office program is superior to patching. Holmes et al., 2016

Does Patching Delay Binocular Vision Development?

Patching covers the stronger eye to encourage use of the amblyopic eye. While the patch is worn, the child cannot practice using both eyes together. Because one eye is covered, it is not directly helpful in development of binocular vision and alignment.

Why Reading and Eye Tracking Matter

Amblyopia can affect everyday visual performance. Research has documented slower reading among children with amblyopia under ordinary, two-eye reading conditions. Kelly et al., 2015

Before-and-after eye-tracking images can help explain how a child’s eye movements change. Reading benefits should also be documented directly through measures such as reading rate and comprehension.

A cleaner tracking pattern can be encouraging, but the recording alone cannot demonstrate improved comprehension or school achievement.
